Accesta Care is a registered NDIS Provider. We provide supports and services to people with disabilities in the Western suburbs of Melbourne, and across Australia. Accesta Care have a range of services to those funded through the National Disability Insurance Scheme (NDIS). 

Support services will be delivered both in-home and outside the home and will include high intensity daily support activities, community nursing, domestic care, transport, community access, post-hospital care services and plan management.

 

Our Mission

Accesta Care seeks to empower participants live life as independently as possible, and offers a variety of attendant care supports to help them achieve their goals. The Participant (or their carer or representative) is matched with the best available Disability Support Worker, either as a one-off service or as part of an ongoing program of care. Our organisation has a plan management expert who help participants understand, and make the most of their NDIS plan.

These supports assistance

Community Nursing

A visit by a qualified nurse in the

your capacity to maintain
relationships, and manage
service delivery tasks.

Assistance with personal
domestic activities

These supports include
assistance with daily
household activities to enable
participants uphold a clean
and healthy living
environment.

Public transport training

This is a program designed to
teach participants how to use
public transport safely and
independently.

Skills development in a
group

Assist in individual life skills
development and training.

Group-based activities

Activities like sports, arts and
crafts, singing and dancing
classes etc.

Social and lifestyle
support

Activities like visiting your
family, going for movies,
meditation or yoga, or active
hobbies like kayaking etc.

Service delivery is centred on clients, and involves them and their supporters in every aspect of their care. Each Participant will have an individually tailored package of care specifically designed to support them to meet their goals, needs and preferences.
